Two recent instances of mouth bacteria causing severe internal infections are reported in the British Medical Journal [2] and also in IFLS [3]

The first instance is probably one of the most well known cases of infection from Streptococcus gordonii bacteria and concerns a woman with a prosthetic knee.


A 65-year-old woman with osteoarthritis, who underwent knee replacement 5 years prior, developed sudden onset knee pain and swelling. She had voluntarily started a vigorous dental floss routine prior to the onset of symptoms.

How could Streptococcus gordonii, a bacterium usually found in the mouth, suddenly cause severe infection in the woman's prosthetic knee after five years with no problems?


After she arrived at the emergency room with pain and swelling around her knee replacement, Doctors eventually identified that her tooth flossing was the culprit due to high levels of bacteria in her mouth.

Her doctors said in their report that it is likely that her vigorous flossing and bleeding gums introduced the bacteria into her bloodstream through tiny cuts in the gums. The bacteria then made their way through the blood stream and settled onto her knee replacement. You can see their report in the BMJ Case Reports.


It was easy for the bacteria to attack the prosthesis, as opposed to elsewhere in the body, because the prosthetic joint had no defence like the rest of the body’s immune system. With an inability to fend off potentially problematic microbes, organisms easily grow on artificial joints. 

To treat the infection, the doctors opened up the woman's knee and washed out as much of the bacteria as they could then prescribed a long-term course of antibiotics. Fortunately, the woman was able to keep her original knee replacement.

This is just one case and the majority of people floss without issue. There is no reason to stop flossing and it is increasingly recommended to oil pull, to maintain manageable levels of bacteria in your mouth.

There have been other cases of a knee joint replacement being infected with S. gordonii. A 78-year-old woman had a relatively similar case in Belgium. She too had swelling around her prosthetic knee from S. gordonii bacteria but she also had swelling in her aorta, an artery in the heart. This patient had her knee prosthesis replaced as well as a valve replacement in her heart. In another instance, Doug Kaye from San Francisco spent most of October 2011 in hospital due to a liver abscess and subsequent complications - all due to mouth infections. 

Doug reports that he was using a WaterPik daily with quite a bit of bleeding from my gums. After five months he became quite ill with an 8cm abscess on his liver. The bacterium was identified as Streptococcus milleri, also typically found in the mouth. Doug's infection spread to the right lung. Thoracic surgery was required to address a pleural effusion. 

What was the cause of Doug's Streptococcus milleri liver infection in the first place?

Once again, this is a bacterium that we all have in our mouths, stomachs and intestines, but it’s toxic in the bloodstream. Doug's team of infectious-disease doctors were extremely inquisitive about his travel, activities, diet and dental history.
